Qualifications

• RN license required.
• Experience leading MDS programs in long-term care.
• Ability to train and mentor MDS Coordinators.
• Knowledge of PDPM, Medicaid/Medicare requirements, and regulatory standards.

Responsibilities

• Lead the MDS program and ensure accurate assessments.
• Determine PDPM and expenses for admissions.
• Monitor CMI and reimbursement risks.
• Coordinate resident assessments and care planning with interdisciplinary teams.
• Provide ongoing education to facility staff and leaders.
• Track and analyze assessment data for compliance and opportunities.
