Job summary

RAM Recruiting is seeking an experienced MDS Coordinator for a well-established skilled nursing facility in Far Rockaway, Queens. This full-time role requires an RN with prior MDS coordination in a SNF setting and offers a competitive salary up to $130,000 per year.

You will coordinate MDS assessments, manage the RAI process, support PDPM reimbursement, ensure regulatory compliance, and work with nursing leadership and the interdisciplinary team to align care plans with residents' clinical

Qualifications

• Current RN license in New York State (required).
• Prior MDS Coordinator experience (required).
• Experience in a skilled nursing facility (SNF) / long-term care (LTC) setting (required).

Responsibilities

• Complete and coordinate MDS assessments on schedule.
• Manage the RAI process and lead care plan meetings.
• Ensure documentation supports PDPM reimbursement.
• Maintain compliance with federal and New York State assessment requirements.
• Collaborate with nursing and therapy teams to align assessments with residents' clinical status.
