Job summary

Smithtown Center is seeking an MDS Coordinator / RNAC to manage MDS data collection and care planning for residents. You will work within our team to ensure accurate assessments and appropriate reimbursement through coordinated processes.

Requirements include RN licensure and prior long-term care experience as an RNC/MDS; you will lead the MDS workflow and collaborate with interdisciplinary teams in a supportive, growth-focused environment.

Qualifications

• Must have previous experience in long term care as an RNC / MDS.
• Must be an RN.
• The MDS Coordinator / RNAC is responsible for completion of Minimum Data Sets (MDS) and associated processes in the development of the individualized plan of care and the appropriate financial reimbursement for each patient.
