RAM Recruiting is seeking an experienced MDS Coordinator for a well-established skilled nursing facility in Far Rockaway, Queens. This is a full-time role offering compensation up to $150,000 per year and requires an RN with prior MDS Coordinator experience in a SNF setting.

You will lead MDS assessments, coordinate the RAI process, and ensure PDPM reimbursement while collaborating with nursing leadership and the interdisciplinary team to support resident care planning.

Formación

• Current Registered Nurse (RN) license in New York State — required.
• Prior experience as an MDS (Minimum Data Set) Coordinator — required.
• Experience in a skilled nursing facility (SNF) / long-term care (LTC) setting — required.

Responsabilidades

• Complete and coordinate MDS (Minimum Data Set) assessments accurately and on schedule.
• Manage the RAI (Resident Assessment Instrument) process and lead care plan meetings with the interdisciplinary team.
• Ensure documentation supports accurate PDPM (Patient-Driven Payment Model) reimbursement.
• Maintain compliance with federal and New York State assessment requirements.
• Work with nursing and therapy teams to keep assessments aligned with residents' actual clinical status.
