Job summary

RAM Recruiting presents a Lead MDS Coordinator (RN) opportunity at a well-established SNF in Brooklyn. You will own the MDS process, guide the MDS team, and work with the DON and interdisciplinary staff to ensure accurate, timely assessments under RAI and PDPM guidelines.

Ideal candidates have RN licensure in NY and previous MDS Coordinator experience in SNF/LTC, with strong leadership and calendar management skills to drive quality measures and survey readiness.

Qualifications

• Active RN license in New York State.
• Prior MDS Coordinator experience in SNF/LTC is required.
• Lead/coordinator team experience strongly preferred.
• Strong working knowledge of MDS, RAI, and PDPM.

Responsibilities

• Lead and support the MDS team, setting priorities and ensuring every assessment is completed and submitted on time.
• Review and audit MDS assessments for accuracy before submission following the RAI manual.
• Own the assessment calendar and track ARDs across the building.
• Maintain PDPM reimbursement accuracy and identify missed opportunities in documentation.
• Train and mentor MDS staff and floor nurses on documentation that supports accurate assessments.
• Coordinate with nursing, therapy, social services, and dietary so care plans reflect each resident's assessment.
• Support the team on quality measures and survey readiness.
