Job summary

Signature HealthCARE of Norfolk is seeking an MDS-focused Registered Nurse candidate to gather data for the MDS, perform bedside assessments, and interface with families. The role emphasizes accurate documentation, collaboration with the interdisciplinary team, and adherence to long-term care regulations.

The ideal candidate has RN licensure, at least three years of clinical experience, and at least one year of MDS experience, with a strong understanding of the regulatory landscape for skilled

Qualifications

• Must be a Registered Nurse with current state licensure.
• Minimum three years of clinical experience in health care, long-term care preferred.
• Minimum one year of MDS experience.
• Knowledge of laws, regulations, and guidelines for long-term care.

Responsibilities

• Collect information to complete the MDS from medical records, bedside assessment and interviews.
• Participate in Daily PPS and monthly meetings to ensure federal billing requirements are met.
• Educate the RAI process to interdisciplinary team members and caregivers.
