Job summary

Signature HealthCARE is seeking an MDS-focused RN in Norfolk to complete resident assessments. The ideal candidate will be detail-oriented, organized, and self-motivated, with current RN licensure and several years of clinical experience.

The role includes collecting MDS data from records and interviews, leading PPS and Medicare meetings, and educating staff on the RAI process to ensure accurate billing and care planning.

Qualifications

• Must hold current state RN licensure.
• Minimum three years of clinical experience in a healthcare setting (long-term care preferred).
• Minimum one year of MDS experience.

Responsibilities

• Collect information to complete the MDS using the medical record, bedside assessment, and staff, resident and/or family interviews.
• Participate in Daily PPS meetings, weekly Medicare meetings, and month end meetings to assure federal billing requirements are met; review Plan of Care quarterly and with each Comprehensive Assessment.
• Provide ongoing education of the RAI process to all interdisciplinary team members, direct and indirect caregivers, business office, etc.
