Job summary

Trinity Manor in Dodge City, KS seeks an RN MDS Coordinator to join our SNF/LTC team in a part-time role. The MDS Nurse coordinates accurate resident assessments, ensuring they reflect needs and conditions in the care plan while collaborating with nursing, therapy, social services, and dietary staff.

Responsibilities include timely MDS completion, care-plan meetings, and documentation reviews to support reimbursement and regulatory compliance. Kansas RN licensure and MDS experience are required.

Qualifications

• Current Kansas RN license in good standing.
• MDS and care-planning experience.
• Knowledge of RAI guidelines, PDPM, and Medicare requirements.
• Strong organizational, communication, and time-management skills.
• Ability to meet deadlines and work effectively with an interdisciplinary team.

Responsibilities

• Complete and coordinate MDS assessments accurately and within required timelines.
• Develop, review, and update individualized resident care plans.
• Coordinate care-plan meetings with residents, families, and the interdisciplinary team.
• Monitor documentation to support resident needs, services provided, and reimbursement.
• Review clinical records for accuracy, completeness, and regulatory compliance.
• Track Medicare and managed-care residents, including assessment schedules and required documentation.
• Participate in utilization review, Triple Check, quality improvement, and compliance activities.
• Educate nursing staff regarding documentation requirements and changes in resident conditions.
• Communicate effectively with physicians, nursing leadership, therapy, residents, and families.
• Assist with audits, surveys, and other regulatory reviews.
• Maintain current knowledge of MDS, RAI, PDPM, Medicare, Medicaid, and applicable regulations.
