Reunion Plaza Senior Care and Rehabilitation Center in Texarkana, TX seeks an RN MDS Coordinator to lead Medicaid/Medicare reimbursement efforts, coordinate MDS and Care Plans, and ensure regulatory compliance. You will support the care team and drive high-quality resident outcomes in a patient‑focused setting.

The role requires an active Texas RN license, 2+ years in a skilled nursing facility, and strong RAI/PPS proficiency, with excellent communication and organizational skills.

Qualifications

• Current RN license in Texas.
• Experience as MDS Coordinator in a skilled nursing facility.
• Proficiency with MDS/RAI PPS processes.

Responsibilities

• Maintain MDS/Care plan schedule for interdisciplinary team.
• Coordinate preparation of MDS and Care Plan per regulatory requirements.
• Track Medicare resident benefit days and coordinate with management for billing.
• Prepare Medicare denial letters within Medicare time frames.
• Participate in Medicaid documentation evaluation.
