Reunion Plaza Senior Care and Rehabilitation Center in Texarkana, TX is seeking an experienced MDS Coordinator (RN). You will focus on Medicaid and Medicare reimbursement, coordinate and complete clinical assessments, care planning, and transmit MDS 3.0 per regulations, ensuring high-quality care.

You will help create a positive patient experience and support our mission of superior clinical care, rehabilitation, wellness, and supportive services for residents.

Qualifications

• Current RN license in the state of Texas.
• Experience completing MDS and Care Plans in an MDS Coordinator role.
• Proficiency with RAI/PPS processes.
• Strong attention to detail and documentation.
• Good communication, clerical, organizational and computer skills.

Responsibilities

• Maintaining MDS/Care plan schedule for interdisciplinary team.
• Coordinating preparation of MDS and Care Plan per regulatory requirements.
• Tracking Medicare resident benefit days and validating daily census with management for accurate billing.
• Completing responses to information requests from the intermediary and appeals decisions within time frames.
• Ensuring preparation of Medicare denial letters for residents discharged from Medicare coverage within required time limits.
• Participating in ongoing evaluation of documentation required for Medicaid reimbursement.
