Job summary

Touchstone Communities in Texas is seeking an experienced RN MDS Coordinator to oversee the MDS assessment and reimbursement process in our skilled nursing facility. You will ensure accuracy, regulatory compliance, and high-quality resident care while supporting financial integrity.

As part of the interdisciplinary team, you’ll lead the MDS cycle for Medicare/Medicaid, collaborate with clinical and financial staff, and stay current with state and federal regulations to maximize reimbursement and

Qualifications

• Active Texas RN license or compact state.
• Strong knowledge of Medicare/Medicaid reimbursement systems.
• Experience in SNF as MDS nurse preferred.
• Detail-oriented with strong organizational skills.
• Ability to collaborate with interdisciplinary teams.

Responsibilities

• Lead the MDS process for accurate reimbursements.
• Coordinate with clinical and financial teams to optimize reimbursement.
• Stay current with state and federal regulations related to RAI and reimbursement.
• Collaborate with interdisciplinary teams to develop individualized resident care plans.
• Ensure accuracy in documentation, assessments, and coding.
