Job summary

Bethesda Health Group seeks a Registered Nurse (RN) to join our long-term care team in Bethesda, MD. You will complete MDS, admissions, changes, discharges and re-entry tracking according to CMS timelines and communicate with the care team to ensure accurate assessments.

Qualifications

Graduate of an accredited nursing program.
Active RN license in the state you will be working.
Two years' experience with long-term care plans, MDS regulations, and CMS-related MDS.
Excellent communication with residents, visitors and staff.
Strong time management and scheduling abilities.
Ability to stand, walk long periods and lift heavy objects safely.
Manual dexterity for patient care and equipment use.
Ability to document electronically and manually.
Ability to work independently with minimal supervision.
Must be a team player.

Responsibilities

Complete MDS, admission, significant change, change of therapy and discharge tracking.
Enter MDS data into patient information systems and transmit to CMS as required.
Ensure 15-month records are available and up to date.
Collaborate with physicians and interdisciplinary team to optimize care plans.
Develop annual care goals coordinating with team members.
Perform additional duties as assigned.
