Job summary

Complete Care at Shorrock Gardens in Brick Township, NJ, is seeking a Registered Nurse - MDS Coordinator to oversee accurate MDS assessments and teamwork across departments to develop resident care plans.

You will ensure documentation supports high-quality care, regulatory compliance, and appropriate reimbursement, while working with an interdisciplinary team to optimize resident outcomes.

Qualifications

• RN license in New Jersey (current, unrestricted).
• Experience as MDS Coordinator or MDS assessment in skilled nursing preferred.
• Strong clinical assessment, communication, and organizational skills.
• Knowledge of federal and state MDS regulations.
• Ability to work with interdisciplinary teams.

Responsibilities

• Coordinate and complete accurate MDS assessments per guidelines.
• Develop individualized resident care plans with nursing, therapy, social services and other departments.
• Ensure documentation supports quality care, regulatory compliance and reimbursement accuracy.
• Monitor MDS assessment schedules and update documentation as conditions change.
• Participate in care plan meetings and contribute clinical expertise to improve resident outcomes.
• Educate and support clinical staff on MDS processes and regulatory updates.
• Collaborate with leadership to maintain compliance and promote quality improvement.
• Help ensure the facility delivers high levels of resident-centered care.
