Job summary

Parham Healthcare & Rehabilitation Center in Richmond, VA is seeking a full-time MDS Coordinator who holds a Registered Nurse license for our 180-bed skilled nursing facility. The typical schedule is Monday–Friday with a weekend rotation as Manager on Duty.

The MDS Coordinator will complete minimum data set assessments and develop comprehensive care plans after reviewing patient records and coordinating with direct care staff, physicians, and family.

Qualifications

• RN license is required.
• Experience in skilled nursing and rehabilitation environment.
• Understanding of the MDS 3.0 process.

Responsibilities

• Complete minimum data set assessments and create plans of care after reviewing medical records and coordinating with staff, physicians and families.
• Develop and complete the patient assessment process per federal/state regulations and company policies.
• Collaborate with interdisciplinary team to ensure regulatory compliance and quality standards.

Parham Healthcare & Rehabilitation Center is seeking a full time MDS Coordinator with a license to practice as a Registered Nurse for our 180bed skilled nursing facility in Richmond.

The typical schedule for the MDS Coordinator is Monday-Friday with a weekend rotation of Manager on Duty.

The MDS Coordinator is responsible for completing minimum data set assessments and creating comprehensive plans of care after review of the patient's medical record and communication with direct care staff, the patient's physician, and family. The MDS Coordinator develops and completes the patient assessment process in accordance with the requirements of federal and state regulations and company policies and procedures.
