Job summary

Oro Valley Hospital is seeking a full-time MDS Assessment Coordinator - RN to oversee the RAI process in the Skilled Nursing unit. You will coordinate MDS timing, ensure accurate documentation, and support interdisciplinary care planning within regulatory guidelines.

Ideal candidates bring 2–4 years of clinical nursing experience and 1–2 years of MDS experience, with strong organizational, communication, and leadership abilities.

Qualifications

• 2–4 years of clinical nursing experience in acute, primary, long-term care, or home health.
• 1–2 years of MDS experience in long-term care preferred.

Responsibilities

• Coordinate MDS assessments for timely completion and submission per state/federal regulations.
• Facilitate interdisciplinary care plan conferences and updates.
• Transmit MDS data to state systems within required timeframes.
• Maintain records supporting compliance and audit readiness.
• Provide updates to private insurance case managers and stakeholders.
• Evaluate admissions to the Skilled Care Unit with interdisciplinary teams.
