Job summary

The MDS Assessment Coordinator - RN at Northwest Healthcare’s Oro Valley Hospital Skilled Nursing Unit oversees the RAI/MDS process, coordinating care plan conferences and ensuring compliant, timely reporting. You will lead nursing staff, liaise with case managers, and support admission evaluations to optimize resident care.

Licensure requirements include RN and BCLS; 2–4 years nursing experience in acute/long-term care; strong EHR and regulatory knowledge are essential.

Qualifications

• 2-4 years of clinical nursing experience in acute, long-term care, or home health.
• 1-2 years of MDS experience in long-term care preferred.
• Strong knowledge of MDS assessments, RAI processes, and regulatory standards.

Responsibilities

• Coordinates scheduling, completion, and submission of MDS assessments per state/federal regulations.
• Facilitates interdisciplinary care plan development and updates.
• Transmits MDS data to state systems within required timeframes.
