Qualifications

• 2-4 years of clinical nursing experience in acute, primary care, long-term care or home health.
• 1-2 years of MDS experience in long-term care preferred.

Responsibilities

• Coordinate MDS assessments scheduling, completion and submission per regulations.
• Facilitate interdisciplinary care plan conferences and updates.
• Ensure timely reporting and regulatory compliance for MDS/RAI processes.
• Lead and support nursing and care staff; ensure audit readiness.
