Job summary

NHC HealthCare Lewisburg is seeking a detail-oriented Registered Nurse to serve as MDS Coordinator RN, coordinating the MDS process and partnering with the inter-disciplinary team in Lewisburg, TN.

Qualified candidates will hold a Tennessee RN license, be a graduate of an accredited nursing program, and have experience with MDS, care planning, and documentation. We offer competitive pay, comprehensive benefits, and a $10,000 sign-on bonus where eligible.

Qualifications

• Current Registered Nurse license in Tennessee.
• Graduate of an accredited school of nursing.
• Experience with MDS, long-term care, or care planning.
• Strong assessment, documentation, organization, communication, and analytical skills.

Responsibilities

• Coordinate and complete MDS assessments according to applicable requirements and timelines.
• Support accurate documentation and interdisciplinary care planning processes.
• Review clinical information and collaborate with nursing, therapy, dietary, social services, and other departments.
• Help ensure assessment data accurately reflects patient and resident needs and services.
• Track required assessment schedules and support timely completion.
• Participate in care-plan meetings and quality improvement activities.
• Maintain compliance with applicable regulations, reimbursement requirements, and NHC policies.
• Provide education and support to team members regarding documentation and MDS-related processes.
