Keswick Multicare Inc. in Baltimore, MD, is seeking an RNAC/MDS Coordinator to implement and evaluate the MDS program and support care planning, ensuring documentation meets reimbursement and regulatory requirements.

Qualifications include RN license in good standing, current BLS, RAC-CT within 6 months, and 3 years as an MDS RNAC. Join a not-for-profit aging services organization focused on connection, empathy, inclusion, and advocacy.

Qualifications

• Registered Nurse (RN) license in good standing.
• Current AHA BLS certification.
• Three years of experience as an MDS Coordinator/RNAC.
• Resident Assessment Coordinator Certified (RAC-CT) must obtain within 6 months of hire.

Responsibilities

• Meets with resident/patient in-person to collect information for the RAI and assess health status.
• Develops nursing diagnoses and plans interventions; communicates with Unit Manager and DON.
• Completes comprehensive resident assessments (initial, quarterly, annual) and ensures proper documentation.
• Completes MDS/MMDS and transmits to the State; contributes to care plan development.
• Educates care-delivery team on MDS information and procedures; supports regulatory surveys.
