Countryside Center for Rehabilitation and Nursing is seeking an RN MDS Coordinator to join our clinical leadership team in Bardwell, KY. You will oversee the MDS assessment process, ensure timely submissions, and collaborate with nursing, therapy, and business office teams to maximize accuracy and compliance.

You will validate documentation, support care plans with precise data, and stay current with RAI and CMS guidelines while training staff as needed.

Qualifications

• Current, active RN license in good standing.
• Long-term care experience preferred, with understanding of resident care and clinical workflows.
• Strong attention to detail and ability to interpret clinical documentation accurately.
• Excellent communication and collaboration skills with interdisciplinary team members.
• Ability to manage deadlines, prioritize tasks, and maintain organized, compliant documentation.
• Willingness to learn MDS/RAI processes, PDPM guidelines, and regulatory requirements.

Responsibilities

• Coordinate and monitor timely completion and submission of all MDS assessments, ensuring compliance with deadlines.
• Collaborate with nursing and therapy leadership to ensure accurate PDPM coding and capture of clinical conditions.
• Track and monitor Medicare and Medicaid assessment schedules and triggers to prevent missed assessments.
• Lead and participate in care plan meetings with measurable outcomes and updated goals.
• Validate documentation for MDS sections reflecting observations and resident status.
• Coordinate with Business Office and Billing to align MDS data with reimbursement needs.
• Maintain up-to-date knowledge of RAI guidelines and CMS updates; train staff as needed.
• Review therapy documentation to ensure consistency with MDS coding.
• Identify trends in documentation and develop corrective action plans with nurse managers.
• Support QAPI initiatives and resident outcomes monitoring.
