Job summary

Cass County Senior Living & Rehabilitation in Virginia, IL is seeking an experienced RN to join the clinical leadership team as MDS Nurse / Infection Preventionist. This role blends MDS coordination with infection prevention to improve resident outcomes and ensure regulatory compliance.

You will work closely with nursing leadership, coordinate accurate assessments, contribute to reimbursement and quality measures, and lead infection control initiatives across the facility, with a Monday–Friday

Qualifications

• Current Illinois RN license required.
• Experience in MDS and infection prevention preferred.
• Long-term care experience is a plus.

Responsibilities

• Coordinate and complete MDS assessments.
• Ensure accurate and timely documentation and care planning.
• Monitor coding, reimbursement, and quality measures.
• Collaborate with interdisciplinary care team on resident outcomes.
• Coordinate the facility's Infection Prevention and Control Program.
• Educate and support staff on infection prevention practices.
