Job summary

Harmony House Care Center is seeking an MDS Coordinator (RN) for day shift, full-time. The role includes developing, monitoring, auditing, and modifying each resident’s care plan to meet individual needs and goals. You will perform resident assessments and assist in discharge planning.

We require an Iowa RN license and CPR certification; prior MDS coordination experience is preferred. CNAs experience is a plus. Harmony House Care Center is an equal opportunity employer.

Qualifications

• Must hold an unencumbered Iowa RN license.
• C.P.R. certification required.
• Prior experience as an MDS Coordinator preferred.
• Experience as RN in a Skilled Nursing Facility and CNAs helpful but not required.

Responsibilities

• Develop, monitor, audit, and modify each resident’s care plan.
• Perform resident assessments and assist in the discharge process.
• Ensure residents receive high-standard care and remain safe and comfortable.
