Whitney Center is seeking an MDS Coordinator to coordinate and expedite MDS/RAI processes for residents. You will ensure compliant assessments, proper coding, and timely submissions while collaborating with the care team to support reimbursement and quality outcomes.

The role requires CT RN or LPN license, RAC-CT certification, and at least 2 years of MDS experience, with strong documentation and interdisciplinary teamwork. This is a full-time position at our Hamden facility.

Qualifications

• Current, active Connecticut RN or LPN license.
• RAC-CT certification.
• Minimum 2 years MDS assessment/coding experience.
• Strong knowledge of MDS 3.0, RAI, OBRA/PPS, and PDPM.
• Excellent clinical assessment, documentation, and organizational skills.

Responsabilités

• Coordinate and complete MDS assessments per regulatory timelines.
• Manage RAI/MDS schedule and ensure timely submission.
• Lead interdisciplinary care planning with nursing, therapy, social services and dietary.
• Review records for accuracy and completeness with supporting docs.
• Ensure documentation reflects resident conditions and needs.
