Suffield House Rehabilitation and Healthcare Center is seeking a Per Diem MDS Coordinator to support a 128-bed facility in Suffield. The role requires RN licensure in Connecticut, CPR certification, and experience in geriatric care. Responsibilities include managing MDS with RAPS, conducting care planning, and participating in Medicare-related meetings.

The position offers flexible hours. Proficiency with MS Office and strong English communication skills are required.

Qualifications

• RN licensed in CT required.
• 3+ years in geriatric nursing/rehabilitation preferred.
• CPR certification.
• Excellent MS Office skills.
• Ability to work flexible hours as needed.

Responsibilities

• Maintain care conferences calendar and conduct interdisciplinary meetings.
• Perform comprehensive resident assessment through chart review and interviews.
• Complete MDS with RAPS and resident summary; develop care plans.
• Ensure timely completion of all RAI processes and transmissions per regulations.
• Lead case management for Managed Care as applicable.
• Participate in CMI and billing meetings as directed by DNS/Administrator.
• Communicate PDPM recommendations as appropriate.
• Review and correct Final validation reports from the State.
• Participate in weekly Medicare meetings and share relevant MDS information.
• Review Medicare resident eligibility for coverage.
• Obtain monthly Quality Indicator Reports and share with DNS/ADNS.
• Perform other duties as directed by the Director of Nursing Services.
• Assure timely completion of resident assessment processes for state/federal agencies.
• Attend meetings as assigned by DNS/ADNS
