Qualifications

Responsibilities

• Gathers data and directs timely MDS 3.0 completion related to Medicare, Medicaid, and Managed Care.
• Collaborates with the interdisciplinary team to address barriers to care.
• Maintains accurate and up-to-date documentation and records.
• Contributes to quality improvement initiatives to enhance care.
• Gathers information, audits ADL logs and assessments for timely MDS completion.
