Absolut Care in Painted Post, NY is seeking an RN Admissions Nurse/RN MDS Coordinator to ensure seamless transitions for residents and coordinate with the care team. This role emphasizes person-centered care and collaboration with interdisciplinary staff.

The ideal candidate will have an active NY RN license, geriatric experience, excellent communication, and the ability to thrive in a fast-paced setting. Hours include 12p-8pm or 11a-7pm shifts with opportunities for involvement in quality

Qualifications

• RN Admissions Nurse/RN MDS Coordinator with an RN license in NY.
• Experience in geriatric care.
• Strong communication and interpersonal skills.

• Compassionate and empathetic approach to patient care.
• Prefer two years’ Medicare/Medicaid experience in long-term care or clinical training.

Responsibilities

• Conduct admissions assessments and develop individualized care plans in collaboration with the care team.
• Coordinate with healthcare professionals and community resources to ensure a comprehensive care approach.
• Provide education and support to residents and their families, promoting independence and empowerment.
• Collaborate with the interdisciplinary team to identify and address potential barriers to care.
• Maintain accurate and up-to-date records and documentation.
• Participate in quality improvement initiatives to enhance the care experience.
• RN Admissions Nurse/RN MDS Coordinator will also be responsible for other duties as assigned by the DON or designee.
• Gathers data and directs timely completion of MDS 3.0 as it relates to Medicare, Managed Care and Medicaid. Monitors, coordinates and enhances the completion of the MDS and the interdisciplinary team members in a professional manner.
• Audits adl logs and assessments to ensure that information is accurate and timely in completing the MDS assessment tool.

Requirements

• RN Admissions Nurse/RN MDS Coordinator with a valid RN license in the state of New York

• Experience in geriatric care

• Prefer two years’ experience in Medicare/Medicaid areas in long-term care or clinical training.
