Job summary

West State Street is seeking a dedicated MDS Coordinator - RN to ensure timely and accurate MDS assessments and to review care plans for physical, cognitive, and social factors in a long-term care setting. You will coordinate interdisciplinary data to support compliant, high-quality resident care.

The role requires an RN license and prior long-term care experience; LPNs with MDS background may be considered. Compensation ranges with benefits are offered in a structured environment.

Qualifications

• Must be a graduate of an accredited School of Nursing with current RN licensure; LPNs with MDS experience may be considered.
• At least one year of long-term care nursing experience is required.

Responsibilities

• Manage the overall process and tracking of all Medicare/Medicaid case-mix documents to assure appropriate reimbursement for services provided within the nursing center.
• Conduct concurrent MDS reviews to assure achievement of maximum allowable RUG categories.
• Integrate information from nursing, dietary, social services, restorative, rehabilitation, and physician services to ensure appropriate reimbursement.
