Qualifications

Responsibilities

• Identifies patient needs through nursing assessment and care planning in collaboration with care team.
• Maintains Care Plan Program including MDS, RAPS, and Comprehensive Care Plans and ensures HCFA requirements are met.
• Acts as regulatory and clinical resource and participates in surveys and quality improvement.
