Job summary

Life Care Centers of America seeks an RN MDS Coordinator to manage and coordinate MDS assessments, CCAs, and care plans in accordance with CMS regulations and Life Care standards. The role supports education for direct care staff and ensures timely submission and accuracy of documentation.

The candidate should have an RN license in good standing, 2 years of nursing experience (geriatrics preferred), and CPR certification. Excellent teamwork and adherence to policies are essential.

Qualifications

• Active RN license in good standing.
• CPR certification upon hire or during orientation.
• Two (2) years’ nursing experience; geriatric nursing preferred.

Responsibilities

• Coordinate and assist with completion and submission of interdisciplinary MDS Assessments, CCAs, and Care Plans.
• Report changes in patient condition identified by MDS to the DON.
• Educate direct care staff on CMS RAI Manual updates and regulations.
• Review Interdisciplinary Comprehensive Care Plans and Final Validation Reports for CMS QIES submission.
• Attend PPS and other required meetings and support documentation accuracy.
