Job summary

Hudson Elms Nursing Home in Hudson, Ohio is seeking an experienced RN MDS Coordinator to join our care team. You will oversee Medicare Data System (MDS) processes, perform comprehensive assessments, and ensure regulatory compliance while maintaining high-quality resident care.

Qualifications

RN license with current registration.
MDS experience and knowledge of regulations.
Excellent communication, organizational, and analytical skills.

Responsibilities

Conduct comprehensive assessments of residents to identify care needs and develop individualized care plans.
Coordinate the MDS process, including data collection, analysis, and reporting for timely submission to regulatory agencies.
Collaborate with interdisciplinary teams to implement effective care plans.
Provide education and training to staff on MDS data collection and reporting requirements.
Participate in quality improvement initiatives to improve clinical outcomes and reduce readmissions.
