The Center for Extended Care at Amherst is seeking a dedicated MDS Coordinator to ensure high-quality care for residents. This role is essential to accurate MDS assessments, care planning, and regulatory compliance within a family-owned skilled nursing setting.

You will collaborate with interdisciplinary teams, communicate with families, and contribute to staff development while upholding a compassionate, patient-centered environment.

Qualifications

• MDS Coordinator certification or equivalent experience.
• Current RN nursing license.
• Solid understanding of MDS assessments and care planning.
• Excellent communication, organizational and interpersonal abilities.
• Ability to work in a fast-paced setting and meet deadlines.
• Familiarity with electronic medical records and computer-based systems.

Responsibilities

• Collaborate with interdisciplinary teams to develop individualized care plans.
• Conduct MDS assessments and identify resident needs.
• Communicate with residents, families, and healthcare professionals for coordination.
• Maintain accurate records and databases to meet regulatory requirements.
• Participate in case conferences and care plan reviews.
• Support staff training and promote a culture of excellence.
