Fulton Manor Care Center in Fulton, MO seeks a dedicated RN to serve as MDS Coordinator, overseeing care planning, assessments, and documentation for residents. You will coordinate with the interdisciplinary team, ensure HIPAA compliance, and maintain accurate MDS records and care plans.

Qualifications

• RN licensure in Missouri.
• MDS Coordinator experience or related healthcare experience.
• Strong communication and interpersonal skills.
• Proficiency with EMRs and healthcare software.
• Ability to multitask in a fast-paced environment.
• Strong organizational and time management abilities.

Responsibilities

• Collaborate with the interdisciplinary team to develop and implement resident care plans.
• Conduct MDS assessments and develop care plans in accordance with regulations.
• Assist with implementing and evaluating resident care programs and services.
• Participate in quality improvement initiatives to improve outcomes and satisfaction.
• Maintain confidentiality and adhere to HIPAA in handling resident information.
• Perform other duties as assigned by the Director of Nursing.
