Job summary

Daytona Beach Health and Rehabilitation Center is seeking a skilled MDS Coordinator (RN) to lead the interdisciplinary resident-assessment process and coordinate MDS assessments and care plans. The role combines clinical assessment with administrative nursing duties to ensure compliance and accuracy in documentation.

The candidate should have an active FL RN license and strong attention to detail, with experience in long-term care or care-planning expected.

Qualifications

• Active Registered Nurse license in Florida.
• Knowledge of professional nursing standards and resident care.
• Experience with MDS, long-term care, or care planning preferred.

Responsibilities

• Coordinate interdisciplinary assessment process for assigned residents.
• Complete and coordinate MDS assessments within required timeframes.
• Coordinate development and completion of comprehensive resident care plans.
• Ensure care plans comply with facility policies and federal/state requirements.
• Monitor resident documentation to keep care plans current.
