Job summary

The Village at Rockville, part of National Lutheran Communities & Services, is seeking an RN MDS Coordinator to lead resident assessments and care planning. You will ensure accurate MDS processes, collaborate with clinical staff, and drive high-quality, resident-centered outcomes in a supportive, team-based setting.

Ideal candidates have Maryland RN licensure, MDS certification or demonstrated coordination experience, and at least two years of clinical work, including a year in long-term care.

Qualifications

  • Current RN license in Maryland.
  • MDS certification or proven experience completing MDS assessments.
  • Minimum 2 years of clinical experience; minimum 1 year in a long-term care setting.
  • Prior experience as an MDS coordinator is strongly preferred.
  • Strong written and oral communication; ability to maintain confidentiality.
  • Proficient in Microsoft Office.
  • Strong organizational and analytical skills with excellent attention to detail.
  • Collaborative spirit and mission-driven service in a senior care community.

Responsibilities

  • Demonstrate commitment to NLCS’ core values in all aspects of your work.
  • Support NLCS policies, procedures, guidelines, and code of conduct standards.
  • Coordinate individualized plans of care using an interdisciplinary team approach.
  • Review and evaluate resident MDS assessments and care plans to ensure proper implementation.
  • Participate in resident care plan conferences.
  • Maintain the MDS schedule within required time frames and communicate pending assessments to relevant directors.
  • Generate and distribute the monthly MDS calendar.
  • Complete and audit MDS assessments, ensuring all required signatures are obtained.
  • Transmit electronic copies of assessments to CMS and reconcile all transmissions.

What We’re Looking For
  • Current RN license in the State of Maryland
  • MDS certification or proven experience completing MDS assessments
  • Minimum 2 years of clinical experience. Minimum of 1 year of experience in a long-term care setting required
  • Prior experience as an MDS coordinator is strongly preferred
  • Must possess strong written and oral communication skills; able to maintain confidential information
  • Must be proficient in Microsoft Office
  • Strong organizational and analytical skills with excellent attention to detail.
