Job summary

Tudor Oaks Senior Living Community in Muskego, WI is seeking an MDS Coordinator (RN) for a full-time, on-site role Monday–Friday 8:00 a.m.–4:30 p.m. The position focuses on leading MDS assessments and care planning to support resident outcomes and compliant documentation.

Requirements include an active RN license (WI or transferable), at least 1 year of MDS nursing experience, strong judgment and communication, and ability to manage deadlines.

Qualifications

• Active, unencumbered RN license (Wisconsin or transferable out-of-state)
• At least 1 year of recent MDS nursing experience
• Strong clinical judgment and attention to detail
• Able to prioritize and manage multiple deadlines
• Effective communication and teamwork skills

Responsibilities

• Coordinate and oversee the interdisciplinary MDS assessment and care planning process
• Ensure timely and accurate completion and transmission of MDS assessments
• Maintain compliance with federal, state, and Medicare regulations
• Collaborate with nursing and interdisciplinary teams to support quality outcomes
• Monitor assessment timelines and address changes in resident condition
• Support documentation accuracy and reimbursement integrity
