Job summary

Complete Care Regent in Hackensack, NJ is seeking an RN MDS Coordinator to lead resident assessments and coordinate care planning. You will guide MDS input, ensure timely completion, and transmit assessments to the state with validated receipts.

The role requires an active NJ RN license and strong knowledge of Case-Mix and Medicare PPS. Experience in LTC/SNF settings is preferred; strong computer skills are essential for daily operations.

Qualifications

• Active NJ RN license required.
• Strong knowledge of Case-Mix, Medicare PPS and medical reimbursement.
• Ability to work independently or in a team.
• Computer literacy.
• Experience as RN MDS in LTC/SNF preferred.

Responsibilities

• Monitor and guide the completion of resident assessments in a timely manner.
• Involvement and supervision of inputting MDS assessments.
• Transmit MDS assessments to the state and obtain receipt of validation.
• Schedule and facilitate care plan conferences on a quarterly and annual basis.
• Other duties as assigned by Director of Nursing and Administrator.

(RN) MDS Coordinator Qualifications:

• Must have an active/valid NJ (RN) Registered Nurse License.

• Strong working knowledge of Case-Mix, Federal Medicare PPS process, and Medical reimbursement.

• Ability to work independently or part of a group.

• Computer knowledge.

• Previous experience working as an RN MDS in a LTC/ SNF setting is strongly preferred.
