Job summary

RAM Recruiting is seeking a dedicated MDS Coordinator (RN) for a well-established SNF in Roselle Park, NJ. This full-time role requires an RN with hands-on MDS experience who knows the MDS process inside out and wants to own the role in a stable building.

You will complete MDS assessments, coordinate with nursing and clinical staff, oversee care plans, monitor PDPM reimbursements, and support survey readiness in a collaborative team environment.

Qualifications

• Active RN license in New Jersey.
• Hands-on MDS Coordinator experience in SNF/LTC setting.
• Experience completing MDS assessments under RAI.
• Familiarity with PDPM reimbursement.

Responsibilities

• Complete and submit accurate MDS assessments under the RAI process, including scheduling and tracking of assessment types.
• Coordinate with nursing, therapy, dietary, and social services to capture the full clinical picture for each resident.
• Oversee care plan development and ensure documentation supports coding.
• Monitor PDPM reimbursement accuracy and Quality Measures, and flag issues before they become problems.
• Support survey readiness through clean, defensible MDS documentation.

A well-established skilled nursing facility (SNF) in Roselle Park, New Jersey is hiring a full-time MDS Coordinator (RN). This is a dedicated MDS Coordinator role, not a floor position with MDS tacked on. The right MDS Coordinator here is a Registered Nurse (RN) who already knows the Minimum Data Set (MDS) process cold and wants a stable building where they can own it.

Prior nursing home experience is required. We are looking for an MDS Coordinator who has actually completed assessments in a skilled nursing / long-term care (LTC) setting and is comfortable with PDPM (Patient Driven Payment Model) reimbursement. What You'll Do
