Job summary

Buena Vida Rehab and Nursing Center in New York is seeking a full-time MDS Coordinator to join our team as an RN with MDS responsibilities.

The ideal candidate will coordinate and complete accurate MDS assessments in accordance with CMS and New York State regulations, review records, and support related reimbursement processes.

Experience with EMR (Point Click Care) and knowledge of MDS 3.0 are a plus; strong assessment and communication skills are essential.

Qualifications

• Current New York State RN license in good standing.
• Long-term care or skilled nursing experience required.
• Some MDS experience preferred; knowledge of MDS 3.0 is a plus.
• Strong assessment, documentation, and organizational skills.
• Excellent communication and teamwork abilities.
• Proficiency with EMR systems (Point Click Care) is required.

Responsibilities

• Complete MDS assessments in compliance with CMS and New York State regulations.
• Ensure accurate and timely completion of MDS, CAAs, and care plans.
• Review medical records to ensure documentation supports MDS coding.
• Support MDS related reimbursement processes.
• Complete Medicare certificates and issue Medicare notices (NOMNCs) in accordance with guidelines.
• Assist with quality improvement initiatives and survey preparedness.

Buena Vida Rehab and Nursing Center is actively seeking a full-timeMDSCoordinator. We are seeking a detail-oriented and compassionate Registered Nurse (RN) to join our team as an MDS Assessor. The ideal candidate will have knowledge of the Minimum Data Set (MDS) process and be responsible for coordinating and completing accurate and timely MDS assessments in accordance with federal and New York State regulations. Previous MDS experience is preferred but not required.

Responsibilities Include

• Complete MDS assessments in compliance with CMS and New York State regulations.

• Ensure accurate and timely completion of MDS, Care Area Assessments (CAAs), and care plans.

• Review medical records to ensure documentation supports MDS coding.

• Support MDS related reimbursement processes.

• Complete skilled Medicarecertificate and issue Medicarenotices (NOMNCs) inaccordance with applicableguidelines.

• Assist with quality improvement initiatives and survey preparedness.
