Cherokee County Health and Rehabilitation Center is seeking an MDS Coordinator to oversee and coordinate resident assessments in accordance with Federal and State rules. You will assist in developing care plans and disseminate updated RAI materials while ensuring timely MDS submissions.

The role requires active licensure in good standing and at least three years of experience in the field, with a focus on resident care planning and family involvement in the process.

• Must be MDS certified and in good standing with state licensing.
• Minimum of 3 years experience in relevant role.
• Experience coordinating resident assessments and care plans.

Responsabilidades

• Oversee and coordinate the development and completion of resident assessments per Federal and State rules.
• Assist in developing comprehensive resident assessments and care plans for each resident.
• Disseminate updated materials involving the RAI process and ensure timely electronic submission of all MDS.
• Create opportunities for family participation in the care planning process.

Oversee and coordinate the development and completion of the resident assessment in accordance with current Federal and State rules, regulations, and guidelines that govern the resident assessment. Assist in the development of a comprehensive resident assessment and care plan for each resident. Disseminate any new or updated materials involving RAI process. Participate in and oversee the timely electronic submission of all MDS. Create an opportunity for family participation in the care planning process.

Type: Full-time Pay: $34.00 per HOUR
