Qualifications

Job Summary And Qualifications

The manager of case management provides supervision and oversees the functional operations of case management and social services. The manager leads a team of clinical professionals to achieve optimal outcomes for patient throughput.

What you will do in this role

• You will provide oversight for the day‑to‑day department activities including but not limited to, education, productivity monitoring, staffing support, communication and scheduling.
• You will have case management, utilization review and/or discharge planning responsibilities.
• You will collaborate with clinicians and support staff in designing and implementing processes to comply with regulatory requirements and assist with TJC survey readiness/preparation.
• You will assist the director with the financial management of the unit, including participation in the annual budget process.
• You will ensure the delivery of quality patient care by analyzing data and initiating and/or participating in performance improvement initiatives.

What qualifications you will need

• (RN) Registered Nurse, or (LMSW/RCSW-I) Lic Master Social Worker/Reg Clin SW-Intern, or Registered Nurse Diploma, or Masters Degree
• 5 years experience Required Years of Experience
