Research Medical Center in Overland Park, KS is seeking a Case Manager to support patients and families throughout the care journey. You will coordinate services and develop care plans reflecting each patient’s needs, partnering with physicians, nurses, and care teams to ensure safe transitions and high-quality communication.

This role emphasizes compassionate, patient-centered care in a hospital setting with a day shift, Monday–Friday, and growth opportunities within the HCA Healthcare system.

Qualifications

• RN with current nursing license.
• Registered Nurse Diploma or equivalent.
• 1 year of relevant experience required.

Responsibilities

• Guide patients and families through orientation, discharge planning, and education on Medicare/insurance benefits.
• Coordinate education for patients, families, and caregivers for treatment planning and family conferences.
• Develop and implement individualized treatment plans reflecting patient needs and recovery goals.
• Complete psychosocial assessments and discharge planning evaluations, documenting findings for the care team.
• Collaborate with Rehab Program Director, Facility Case Management Director, and interdisciplinary team to ensure seamless care.
• Promote compassionate, respectful care aligned with HCA Healthcare values in every interaction.

Job Summary and Qualifications

As a Case Manager, your role will be to support patients and families through every step of their care journey. You will coordinate services, connect resources, and develop care plans that reflect each patient's unique needs. By partnering with physicians, nurses, and department leaders, you will help ensure safe transitions, clear communication, and consistent quality across the continuum of care.

Your responsibilities will include:

• Guiding patients and families through program orientation, explaining the rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights
• Coordinating education for patients, families, and caregivers to encourage participation in treatment planning, goal discussions, and family conferences
• Developing and implementing individualized treatment plans that reflect the patient's strengths, needs, and personal recovery goals
• Completing psychosocial assessments and discharge planning evaluations, clearly documenting findings and communicating needs across the care team
• Collaborating closely with the Rehab Program Director, Facility Case Management Director, and interdisciplinary team to ensure seamless, patient-centered care
