Summary of Job Duties:

The Registered Nurse (RN) will provide direct patient care, coordinate care, triage patients, and collaborate with healthcare providers. Responsibilities include working with patient referral sources, facilitating appointments, coordinating transitions of care, and patient education on complex conditions. The RN develops and implements individual health plans with patients, families, and care teams.

Qualifications:

Minimum Qualifications:

• Graduate of an accredited Nursing Program with an Associate's Degree, Nursing Diploma, Bachelor's, or Master's in Nursing
• Must hold a valid RN license
• At least 1 year of RN experience, with critical care/ED/ICU experience required
• Basic computer skills
• Excellent communication skills
• Strong work ethic and accountability

Preferred Qualifications:

• Current BLS certification
• ACLS certification (or obtain within one year)

Additional Information:

Employees must be fitted for and capable of donning PPE, including N95 masks, PAPR, or CAPR.

• Assess and document patient data

• Administer medications following standards and regulations
• Ensure care delivery aligns with patient and family-centered principles
