The Crescent in Sugar Land, TX is hiring a full-time LVN/LPN Charge Nurse to lead and oversee patient care within the licensed scope. You will monitor patient conditions, document changes, and ensure accurate EMR entries and adherence to care plans.

Collaboration with CNAs and adherence to state guidelines are essential. Candidates must graduate from an accredited nursing program and hold an active LVN/LPN license.

Qualifications

• Graduation from an accredited Nursing School.
• Valid LVN/LPN license required.
• Experience in patient care and clinical supervision.

Responsabilités

• Participate in and oversee patient care within the scope of LVN/LPN licensure.
• Observe, assess, and report patient conditions and document changes per policies and regulations.
• Document in EMR and ensure care plans are implemented and medications administered.
• Supervise CNA staff to ensure high-quality clinical care.

Qualifications: Responsibilities:

• As an LVN/LPN - Licensed Vocational Nurse or Licensed Practical Nurse, you will participate in and oversee patient care within the scope of the LVN/LPN licensure.
• Observes, assesses, and reports Patient conditions/changes and documents in accordance with Cantex’s Clinical Policies & Procedures and state/federal guidelines & regulations.
• EMR documentation relevant to patient assessment, evaluation, change of condition, in accordance to nursing practices.
• Ensures implementation of care plans as directed and administers medications and treatments as prescribed.
• Participates in assigning duties and supervises the delivery of appropriate clinical care by the CNA staff to ensure highest practical well-being is maintained by patient.
