Job summary

Cardea Health in Oakland, CA seeks a Case Management Manager (LVN or RN) to supervise Care Coordinators across interim housing programs, strengthening care coordination, documentation, and quality benchmarks. The role supports residents with complex needs, coordinates across healthcare partners, and leads discharge and transition practices to improve stability and health outcomes.

Travel between sites is required, and the position emphasizes trauma-informed, person-centered, and harm-reduction

Qualifications

• High school diploma or equivalent required.
• At least 2 years in case management, care coordination, or related human-services setting.
• At least 1 year of experience supervising staff.

Responsibilities

• Provide direct management, supervision, coaching, and performance oversight for Care Coordinators across program sites.
• Establish standardized care-coordination practices, workflows, documentation expectations, and service-quality benchmarks across all locations.
• Lead interdisciplinary case reviews and facilitate coordinated, resident-centered action plans that clarify team roles, priority interventions, follow-up responsibilities, timelines, and escalation pathways.
• Oversee standardized discharge and transition-of-care practices for residents exiting Cardea programs.
