60-bed facility, roughly 52 census pcc for charting 1:25-30 for ratios.
Provide high-quality nursing care for LTC residents adhering to care plans.
Assess resident condition, document changes, and communicate with care team promptly.
Administer medications and monitor for adverse reactions per protocols.
Ensure compliance with regulatory standards and facility policies at all times.
Develop, implement, and update individualized care plans with measurable goals.
Collaborate with physicians, therapists, and family members to optimize outcomes.
Track metrics and outcomes to meet performance and quality targets.
Document and report safety incidents and participate in root-cause analyses.
