Thurs - Sat OT available, especially if they wanna pick up any day shifts

60-bed facility, roughly 52 census pcc for charting 1:25-30 for ratios.

Responsibilities

Provide high-quality nursing care for LTC residents adhering to care plans.

Assess resident condition, document changes, and communicate with care team promptly.

Administer medications and monitor for adverse reactions per protocols.

Ensure compliance with regulatory standards and facility policies at all times.

Develop, implement, and update individualized care plans with measurable goals.

Collaborate with physicians, therapists, and family members to optimize outcomes.

Track metrics and outcomes to meet performance and quality targets.

Document and report safety incidents and participate in root-cause analyses.

Mentor and educate junior staff to promote professional growth and performance.

Assist with admissions, discharges, and transfers ensuring continuity of care.

Utilize care pipelines and territory insights to identify opportunities for improvements.

Experience/Qualifications

Active RN license in the applicable state with LTC experience.

Proven ability to develop and update individualized care plans with measurable goals.

Experience administering medications and monitoring adverse reactions per protocol.
