Responsibilities

Provide patient care and basic nursing tasks within LTC setting per protocols.

Monitor patient status; document vitals, changes, and care plans accurately.

Administer medications and treatments as prescribed; track adherence and side effects.

Ensure compliance with safety, infection control, and regulatory standards.

Collaborate with an interdisciplinary team to update care plans and discharge planning.

Identify and report clinical changes early; elevate to supervising RN as needed.

Demonstrate accountability for personal performance; meet individual targets and quality metrics.

Support professional development through ongoing learning and skill enhancement.

Maintain organized patient records, optimize workflow, and improve care continuity.

Engage with residents and families to provide compassionate communication and education.

Participate in daily huddles and pipeline reviews to forecast care needs and staffing.

Assist with monitoring pipelines for potential admissions and transfers within the territory.

Experience / Qualifications

Current LPN license with active‑practicing permission.

Proven LTC or long‑term care nursing experience preferred.

Ability to document vitals, care plans, and medications accurately.

Competence in medication administration and monitoring for side effects.

Strong clinical assessment skills and early escalation to RN when needed.

Knowledge of safety, infection control, and regulatory standards.

Excellent communication with residents, families, and interdisciplinary team.

Ability to work in a performance‑driven, metrics‑focused environment.

Proactive participation in daily huddles, pipeline reviews, and staffing planning.

On‑site availability with reliable time management to meet targets.
