Waverly Rehabilitation And Healthcare Center is hiring a Wound Care Nurse (FT) to lead individualized wound care plans in collaboration with a multidisciplinary team. You will preserve skin integrity, ensure protocol approval by medical staff, and participate in rounds for high-risk patients.

Responsibilities include hands-on wound care for challenging wounds, organizing restorative activities, and maintaining compliance with regulatory standards.

Qualifications

• Must have active RN or LPN license.
• LTC/SNF experience preferred.

• Preserve skin integrity by implementing skin preservation strategies.
• Participate in weekly wound and skin rounds for high-risk patients and others with wounds.

• Ensure regulatory requirements are met.

Wound care Nurse F/T
Summary/Objective:
In this role, you will develop and ensure the wound care plan specific for each patient in collaboration with an interdisciplinary team.

• Preserving the integrity of the skin by implementing and monitoring skin preservation strategies.
• Ensuring all wound and skin care protocols are approved by the medical staff.
• Participating multidisciplinary wound and skin rounds on a weekly basis for high risk patients and patients with current pressure ulcers and other significant wounds.
• Providing hands-on wound care for all difficult and non-healing wounds.
• Organize and implement the restorative program.
• Ensuring all regulatory requirements are met.

Qualifications:

• Current State License as an RN/LPN
• LTC/SNF experience preferred
