Job summary

Royal Megansett Nursing and Rehabilitation Center is seeking an RN/LPN Unit Manager to lead nursing care on the unit and supervise the nursing staff. You will develop care plans, administer medications, and collaborate with a multidisciplinary team in a memory care-focused setting.

You will uphold high standards of resident-centered care, ensure compliance with HIPAA and regulations, and support residents and families through coordinated discharge planning and education.

Qualifications

• RN or LPN licensed in this state.
• Minimum 2 years of nursing experience.
• Previous leadership or supervisory experience.
• Experience administering medications.
• Proficient with PCC EMR.
• Knowledge of memory care protocols.
• Familiarity with MDS assessments for long-term care.
• Experience with case management in SNF/sub-acute settings.

Responsibilities

• Plan, organize, direct and evaluate nursing care on the unit.
• Provide supervision to unit staff.
• Ensure assignments match staff competencies and patient needs.
• Assess resident health, develop and implement care plans.
• Administer medications and treatments as prescribed.
• Monitor progress and respond to changes in condition.
• Collaborate with multidisciplinary teams.
• Educate residents and families on health plans and discharge.
• Maintain accurate records in compliance with HIPAA and regulations.
• Participate in case management for continuity of care.
• Support in specialized areas such as PT/OT/SUD and pain management.

RESPONSIBILITIES

Including, but not limited to:

• Plans, organizes, directs, supervises and evaluates nursing care provided on the unit to achieve clinical and financial outcomes.

• Provides professional guidance and supervision to non-professional unit staff.

• Ensures patient care assignments are consistent with staff competencies, scope of practice and the needs of the patients.

• Assess resident health problems and needs, develop and implement nursing care plans.

• Administer medications and treatments as prescribed by physicians.

• Monitor resident progress and respond to changes in condition.

• Collaborate with multidisciplinary teams to provide comprehensive care.

• Educate residents and their families regarding health conditions, treatment plans, and discharge.

• Maintain accurate resident records and documentation in compliance with state and federal regulations, while ensuring compliance with HIPAA regulations.

• Participate in case management activities to ensure continuity of care.

• Provide support in specialized areas such as PT, OT, SUD, Pain Management, etc.

SKILLS

• Registered or Licensed Nurse in this state

• 2 years or more of nursing experience

• Leadership experience

• Medication administration

• EMR: PCC (Point Click Care)

• Proficiency in memory care techniques and protocols.

• Knowledge of MDS (Minimum Data Set) assessments for long-term care facilities.

• Familiarity with operations in Skilled Nursing, Sub-Acute, and Dementia Units.

• Strong background in case management practices.

• Ability to work effectively in a medical office environment.

• Excellent interpersonal skills for effective communication with patients, families, and healthcare teams.

• Strong organizational skills to manage multiple tasks efficiently.
