Job summary

AuthentiCare Health Centers is seeking an RN/LPN Unit Manager to oversee nursing care on the unit and lead a dedicated team in a skilled nursing environment. The role focuses on resident-centered care across long-term, sub-acute, and memory care settings.

Strong clinical skills, leadership and adherence to regulatory standards are essential. You will supervise staff, coordinate care plans, administer medications, and ensure accurate documentation while collaborating with a multidisciplinary team

Qualifications

• Registered nurse licensed in this state.
• Minimum 2 years nursing experience with leadership exposure.
• Experience using EMR PCC and memory care protocols.

Responsibilities

• Plans, directs, and evaluates nursing care on the unit to achieve clinical and financial outcomes.
• Provides professional guidance and supervision to unit staff.
• Ensures assignments align with staff competencies and patient needs.
• Assess resident health problems, develop and implement care plans.
• Administer medications and treatments as prescribed.
• Monitor resident progress and respond to condition changes.
• Collaborate with multidisciplinary teams for comprehensive care.
• Educate residents and families on health conditions, plans, and discharge.
• Maintain HIPAA-compliant resident records and documentation.
• Participate in case management to ensure continuity of care.
• Support in specialized areas such as PT, OT, SUD, Pain Management.

RESPONSIBILITIES

Including, but not limited to:

• Plans, organizes, directs, supervises and evaluates nursing care provided on the unit to achieve clinical and financial outcomes.

• Provides professional guidance and supervision to non-professional unit staff.

• Ensures patient care assignments are consistent with staff competencies, scope of practice and the needs of the patients.

• Assess resident health problems and needs, develop and implement nursing care plans.

• Administer medications and treatments as prescribed by physicians.

• Monitor resident progress and respond to changes in condition.

• Collaborate with multidisciplinary teams to provide comprehensive care.

• Educate residents and their families regarding health conditions, treatment plans, and discharge.

• Maintain accurate resident records and documentation in compliance with state and federal regulations, while ensuring compliance with HIPAA regulations.

• Participate in case management activities to ensure continuity of care.

• Provide support in specialized areas such as PT, OT, SUD, Pain Management, etc.

SKILLS

• Registered or Licensed Nurse in this state

• 2 years or more of nursing experience

• Leadership experience

• Medication administration

• EMR: PCC (Point Click Care)

• Proficiency in memory care techniques and protocols.

• Knowledge of MDS (Minimum Data Set) assessments for long-term care facilities.

• Familiarity with operations in Skilled Nursing, Sub-Acute, and Dementia Units.

• Strong background in case management practices.

• Ability to work effectively in a medical office environment.

• Excellent interpersonal skills for effective communication with patients, families, and healthcare teams.

• Strong organizational skills to manage multiple tasks efficiently.
