Job summary

AuthentiCare Health Centers is seeking an RN/LPN Unit Manager to lead nursing care on the unit and supervise staff. The role focuses on delivering high-quality resident-centered care across long-term, sub-acute, and memory care settings.

Qualifications

• Active RN or LPN licensure in Massachusetts.
• Minimum 2 years of nursing experience; leadership experience preferred.
• Experience in long-term care, sub-acute, or memory care is desirable.

Responsibilities

• Plan, organize, direct, supervise, and evaluate nursing care on the unit.
• Provide professional guidance to staff and ensure appropriate assignments.
• Administer medications and treatments as prescribed.
• Monitor resident health and adjust care plans as needed.
• Collaborate with multidisciplinary teams to deliver comprehensive care.
• Educate residents and families about health conditions and discharge plans.

RESPONSIBILITIES

Including, but not limited to:

• Plans, organizes, directs, supervises and evaluates nursing care provided on the unit to achieve clinical and financial outcomes.
• Provides professional guidance and supervision to non-professional unit staff.
• Ensures patient care assignments are consistent with staff competencies, scope of practice and the needs of the patients.
• Assess resident health problems and needs, develop and implement nursing care plans.
• Administer medications and treatments as prescribed by physicians.
• Monitor resident progress and respond to changes in condition.
• Collaborate with multidisciplinary teams to provide comprehensive care.
• Educate residents and their families regarding health conditions, treatment plans, and discharge.
• Maintain accurate resident records and documentation in compliance with state and federal regulations, while ensuring compliance with HIPAA regulations.
• Participate in case management activities to ensure continuity of care.

• Provide support in specialized areas such as PT, OT, SUD, Pain Management, etc.

SKILLS

• Registered or Licensed Nurse in this state
• 2 years or more of nursing experience
• Leadership experience
• Medication administration
• EMR: PCC (Point Click Care)
• Proficiency in memory care techniques and protocols.
• Knowledge of MDS (Minimum Data Set) assessments for long-term care facilities.
• Familiarity with operations in Skilled Nursing, Sub-Acute, and Dementia Units.
• Strong background in case management practices.
• Ability to work effectively in a medical office environment.
• Excellent interpersonal skills for effective communication with patients, families, and healthcare teams.
• Strong organizational skills to manage multiple tasks efficiently.
